Passport Re-Application
A Passport Re-Application is the most rare type of passport application. If you were just issued an adult passport within the last year, AND your name has since legally changed, then you may apply for a passport re-application. If your name has changed but your passport is over one year old, you must then apply for a passport renewal.
Is a Passport Re-Application what you need? Ask yourself the following question:
- Was your passport issued within the last year?
- Since you passport was issued in the last year, has your name legally changed?
If you can answer YES to BOTH questions, a Passport Re-application is what you need. Please print Part 1 and Part 2 below.
(Please Note: If your passport was issued more than one year ago, and is still valid – you need a Renewal Application)
Part 1 is the Passport Re-Application Instructions, Part 2 is the government passport application. Both parts are required and must be printed and completed in ink.
