New Passport: Is this what you need? Ask yourself these questions:
Am I 16 or older and applying for a passport for the first time?
Was my prior passport issued at age 15 or under?
Was my prior passport issued more than 15 years ago?
If you answer yes to (Any) of these questions, then a New Passport is what you need.
Passport Renewal: Is this what you need? Ask yourself these questions:
Was my prior passport issued when I was age 16 or older, (And)
....the passport is still in my possession and undamaged, (And)
....the passport was ISSUED less than 15 years ago?
If you answer yes to (ALL) of these scenarios, then a Passport Renewal is what you need.
Child Passport: Is this what you need? Ask yourself these questions:
Are you age 15 or younger?

Lost or Stolen Passport: Is this what you need? Ask yourself these questions:
Has my passport been stolen? Have I lost my passport?

Damaged Passport: Is this what you need? Ask yourself these questions:
Has my passport been Damaged?
If you answer yes to this question, then a Damaged Passport is what you need.
Passport Re-Application: Is this what you need? Ask yourself these questions:
Was your passport issued within the last year?
If yes - you need a Re-Application Passport
If your passport was issued more than one year ago, and is still valid - you need a Renewal Application.
Extra Visa Pages: Is this what you need? Ask yourself this question:
Do you have a valid U.S. Passport that needs extra visa pages?
If you answer yes to this question, then extra visa pages is what you need.
