If your passport is still valid and you need more blank visa pages within your passport, then an Extra Visa Pages application is what you need.

• Are you in need of additional blank visa pages in you valid passport?

If you answer yes to this question, then please print part 1 and part 2 below.

Part 2 – How to Complete:

• Go Past the privacy disclaimer – and click “Add Visa Pages” on the following page
• Enter all information requested by State Department (red asterisk fields are required)
• “Summary of Fees” page (only check $60 Expedite Fee) & click SUBMIT
• Next Page – Scroll to bottom and click CREATE FORM
• Your completed Passport Application form will open as a PDF document, PRINT IT OUT & SIGN IT
