New Passports are for first time passport applicants age 16 or older, and also for adults that have had a passport before, but that passport expired over five years ago.

If you think you need a New Passport – Ask yourself the following:

  • Am I applying for a passport for the first time and I am 16 or older?
  • Was my prior passport issued more than 15 years ago?

If you can answer YES to any of these question, then a New Passport is what you need.  Please print Parts 1 and 2 below.  Part 1 can be printed and filled out by hand in ink.  Part 2 will take you to the U.S. State Department’s website, where you are required to complete the passport application electronically and then print it out once completed.  Please see below for detailed instructions on how to complete Part 2.

Passport Renewals are only for adults that currently have an adult passport (10 year validity) in their posession. If you currently have an adult passport OR your adult passport has expired within the last five years, then a passport renewal is what you need. If your passport is damaged in any way you cannot apply for a renewal, you must apply for a damaged passport.

Do you need a Passport Renewal? Ask yourself the following questions:

  • Was your passport valid for 10 years?
  • Was your passport issued within the last 15 years?
  • Were you 16 or older when your most recent passport was issued to you?

You must answer YES to all of the above questions in order to be eligible of a Passport Renewal. Please print instructions 1 and 2 below.

If you have lost your adult passport (10 year validity) then a lost passport application is what you need. You will need to submit either a birth certificate or an older passport when applying for this type of application.

Is a Lost Passport Application what you need? Ask yourself the following questions:

  • Have I lost my passport?
  • Has my passport been stolen?

If you are able to answer YES to ANY of the above questions, a Lost Passport application is what you need. A Child Passport Application is for anyone applying under the age of 16 years old. Even if your child has had a passport before, your child still should apply for a child passport. There is no such thing as a renewal for a child.

Is a Child Passport application what you need? Ask yourself the following question:

  • Are you under the age of 16?

If you are age 15 and Under, you need to apply for a Child Passport. Please Print Parts 1, 2, and 3 below. Second Passports are issued by the US State Department only in certain specific situations. If you travel overseas very frequently you may need a second passport to handle the visa processing in one passport while simultaneously traveling on the second passport. Another reason is for Middle East travel. If you have an Israeli stamp on your passport you cannot travel to many Arab countries, therefore you can obtain a second passport for traveling to those Arab countries. You must document clearly why your need is warranted for this type of application.

Is a Second Passport Application what you need? Ask yourself the following questions:

  • Do I have an Israeli stamp on my passport and need to travel to an Arab country with Israeli stamp restrictions?
  • Do I travel so much that I have one passport getting visas approved while I need another passport to travel with?
